Rarely on the broadest terms, and you should not spend your budget trying. A hospital domain carries authority you cannot buy. What it cannot do is publish a page written for one condition in one town and keep it current, because that content has to clear a committee. A page on fatty liver treatment in your city can outrank all of them for that search, and enough of those pages will out-earn one fight you were never going to win.
It matters more than it used to, because the referral is a name and the patient still searches it. What they find decides whether they call you or ask for someone else. Each one gets its own page and its own Google Business Profile, including the surgery center, which is a separate entity as far as Google is concerned. The common failure here is a Locations page with an address list on it, which ranks in none of those towns. Each office needs a page written for its own area, with the doctors who actually work there.
It is content Google holds to its strictest standard, which makes it an advantage rather than a risk if you handle it properly. That means a named physician as author, a clear review date, no diagnosis offered on the page, and no claim you would not make in a first appointment. Done that way it is some of the highest-trust content on your site.
Local and Google Business Profile improvements often move within the first month or two, and screening searches respond early because intent is high. Competitive condition terms take longer, usually four to six months before the trend is clear. Any agency promising page one in thirty days is either targeting terms nobody searches or is not being straight with you.
It is seldom about effort. Most practices fill in every field, collect reviews and still stay outside the top three, because the map pack runs on signals owners rarely manage: the right primary category, a profile that matches the site, one consistent name and address across the health directories, and steady review activity.
